Evaluation of topical ketorolac on the inflammation and anterior segment complications after cataract surgery in pseudoexfoliation syndrome

Abstract: The goal of this study was to compare the anti-inflammatory effect of ketorolac drop added to a topical steroid versus a topical steroid alone in patients with PEX after cataract surgery.

Methods: 88 Patients with cataract and clinical signs of PEX, from Sep 2017 to Apr 2019, who underwent phacoemulsification in Amiralmomenin Hospital, Rasht, Iran, were randomized into two groups: group 1 who received Betamethasone 0.1% and chloramphenicol 0.5% ophthalmic solutions after surgery and group 2 who received ketorolac tromethamine 0.5% ophthalmic solution in addition to mentioned. SUN system grading was used to assess the severity of inflammation in anterior and posterior chambers; and standard grading for anterior capsular opacity (ACO) and posterior capsular opacity (PCO). Age, sex, BCVA (Best Corrected Visual Acuity), and IOP (Intraocular Pressure) also were compared between the two groups.

Results: BCVA was better in Group 2 than Group 1 at 3, 7, 30 days after surgery but it was not statistically significant (P-value>0.05). The mean IOP difference between the two groups was statistically significant at one month follow-up time (P-value<0.05) and borderline at 6 month follow-up time (P-value: 0.055). The severity of posterior and anterior capsular opacities was higher in Group 1 than in Group 2 at 6 month follow-up time (P-value<0.05). The severity of inflammation was lower in Group 2 in comparison with Group 1 at 3, 7, 30 days of follow-up time (P-value<0.05). All data were analyzed by SPSS v21.0.

Conclusion: Adding ketorolac drop to topical steroids after cataract surgery in eyes with PEX was associated with a greater reduction in the severity of inflammation in comparison with steroid drop alone, and so is recommended to be used after cataract surgery in these patients.
