Hypotropic Dissociated vertical Deviation : a report of three similar cases and surgical management

Abstract: A rare variant of Dissociated vertical deviation (DVD), that has been termed hypotropic DVD, is opposite the typical DVD and almost always shows different characteristic and treatment approach. Three patients are presented who had this rare variant of DVD, Two of them underwent surgery and had good outcomes

Methods: Three consecutive cases of hypotropic DVD and Data regarding age, visual acuity, laterality, amount of deviation, and type of surgery were analyzed.

Results: Mean age at diagnosis was 36 years (range, 32 to 42 years). All patients had unilateral hypo DVD. The average amount of hypotropia was 26 prism(range 20-35 diopters). All patients had severe monocular vision deficits ranging from counting finger to 20/200. All patients had high myopia. A large recession of the inferior rectus muscle was performed in two patients . The results of surgery were satisfactory. One patient denied to do surgery.

Conclusion: Dissociated hypotropia is a rare form of the dissociated strabismus complex. Hypotropic DVD is mostly unilateral and commonly is associated with monocular visual defificits or high myopia. This rare condition can be corrected surgically.
